I consent to my child’s participation in various classroom field trips through the current school year. My child is in good health and has no physical condition that would prevent participation. I understand that some field trips may include my child being transported on a bus, driven by a staff member, or another parent. I understand that the Grace Family Learning Center will endeavor to give me at least 48 hours notice of all field trips.

I voluntarily and willingly assume responsibility for all risks and dangers associated with my child’s participation in the field trips. In consideration of my child’s participation, I waive all claims or causes of action against Grace Family Learning Center, its officers, directors, employees and agents, arising out of my child’s participation in the field trip, and hereby release, hold harmless and discharge Grace Family Learning Center and is officers, directors, employees and agents from all liability..
